Chemical / Radiation Accident
In the event of a serious chemical or radiation spill or accident, call 911,
or
report the circumstances to the Occupational Safety & Environmental Health Office, 231-7759.
- Be prepared to give specifics: i.e. chemical/radioactive material, building name, room number, person(s) injured, etc.
- If necessary, evacuate the building by activating the fire alarm. Refer to building evacuation instructions posted in the building.
- After the emergency is under control, fill out the ND Incident Report Form (ND Risk Management Fund SFN 50508), and be certain others who assisted also fill out a copy.
- Send one copy of the ND Incident Report Form to the University Attorney, one copy to your immediate supervisor, and one to the Library Director.
